This digital letter form serves to confirm your request and agreement regarding the early termination of your contract with Musipire Music School.
As per our contract, terminating the agreement before the end date incurs a penalty fee of $800. Typically, this fee is paid separately by the teacher after receiving their final payments. However, you have expressed a preference to have this penalty fee deducted directly from your final paychecks.
To proceed with this arrangement, please read and agree to the terms below:
- Early Termination Fee: You acknowledge that terminating your contract early incurs a penalty fee of $800, as outlined in your original agreement.
- Deduction from Final Payments: You authorize Musipire Music School to deduct the $800 penalty fee directly from your remaining payments. If the total amount owed to you does not cover the full $800 penalty in a single paycheck, the remaining balance will be deducted from any subsequent payment(s).
- Additional Payment if Needed: If the final payments do not cover the total $800 penalty fee, you agree to pay Musipire Music School the remaining balance to make up the difference.
- No Further Claims: By signing this agreement, you confirm that this arrangement is made at your request and with your full consent. You agree not to pursue any further claims or disputes regarding this deduction.
By completing this digital form and signing below, both you and Musipire Music School will receive a copy of this agreement, ensuring mutual acknowledgment of these terms.
Acknowledgment and Agreement
I have read and understood the terms outlined in this letter. I agree to have the $800 early termination fee deducted from my final payments as described above and will pay any remaining balance if my final payments do not cover the full amount.
